Care Coordinator

The Perfectly Flawed Foundation, a 501c3 nonprofit aimed at addressing the substance use and community health needs with innovative strategies, has an immediate opening for a our Mobile Wellness & Support program.

Position Summary: 

The position will work in coordination with The Perfectly Flawed Foundation’s Mobile Wellness & Peer Support program to conduct needs assessment, service planning, and linkage to services for individuals and families impacted by substance use. The role will travel with Mobile Wellness staff as needed and conduct SBIRT screenings (Screening, Brief Intervention and Referral to Treatment) and service navigation for program participants in LaSalle, Bureau and Putnam county region.  The position will serve as a mentor, advocate and motivator to individuals in order to improve their health and wellness, live a self-directed life, and strive to reach their full potential.

  • Complete formal assessment of service recipient’s strengths and needs using SBIRT screening protocol and facilitate appropriate referrals and connections to services based on screening. 

  • Develop a detailed formal plan for connecting service recipients to needed healthcare (physical and behavioral), housing, employment opportunities or services, food pantry access, transportation, etc. 

  • Advocate on the individual’s behalf to obtain necessary and appropriate services

  • Assist service recipients in obtaining valid identification and medical insurance

  • Work collaboratively with Mobile Wellness and Peer Support staff to ensure services are appropriate for the service recipients needs.

  • Perform non-intrusive follow up with program participants 

  • Develop relationships with local service partners, including providers of medications and other treatments for opioid and substance use disorder

  • Maintain complete and accurate files on all participant programs

  • Participate in rotation of “on-call” monitoring of 24/7 hotline

  • Participate in team staffing calls, continuing education and training activities.

  • Maintain project records in appropriate files and databases

  • Maintain complete and accurate files on all client programs


Necessary skills/qualifications: 
  • Experience in the Behavioral Health field that indicates knowledge of mental health and substance use disorder

  • Any combination totaling two years work experience which provided knowledge of human behavior, including the ability to cope with a variety of people and/or understand the importance of people’s needs and feelings, in a public/customer service environment. 

  • Respects and understands the importance of prioritizing the protection of confidentiality of service recipients

  • Willingness to embrace and gain understanding of multiple pathways to recovery

  • Working knowledge of services provided by various social service agencies. 

  • Skilled in interviewing individuals and families to determine their physical and social needs. 

  • Ability to provide support and encouragement using a non-judgmental perspective to work with program participants “where they are at”.

  • Enthusiastic about providing support to persons in early recovery from substance use disorder

  • Ability to organize, prepare and maintain records, reports, files and correspondence 


Preferred skills/qualifications 
  • Associate Degree, Bachelor's Degree or higher

  • CADC, CRSS or Recovery Support Specialist certification

  • Prior experience in motivational interviewing, WRAP Planning (Wellness Recovery Action Plan) or Crisis Intervention Training.

  • Knowledge or experience with Self-Management And Recovery Training (SMART)

  • Willingness to participate in an on-the-job training. 

  • Basic understanding of harm reduction framework

  • Self motivated in gaining knowledge of innovative approaches to substance use, harm reduction, wellness and recovery

  • Knowledge of stressors that can impact patients in early recovery from opioid use disorder.

  • Knowledge of medications used to treat opioid use disorder. 

  • Ability to work with a myriad of patients from various racial, ethnic and socioeconomic backgrounds. 

  • Ability to work independently, exercise good judgment, and make sound decisions 

  • Ability to adapt quickly to a rapidly changing environment and growing organization

  • Ability to build relations with peer recovery coaches and other members of the healthcare field.  

  • Personal experience in recovery from a substance use disorder or supporting a family member through substance use disorder recovery.   

  • Ability to translate (English/Spanish) for patients and others as necessary would be beneficial however is not a requirement.


Salary Range: Negotiable and Based on Experience

Kyle Petersen

Kyle Petersen graduated from LaSalle-Peru Township High School in May of 2020 and currently is attending Olivet Nazarene University where he studying Multimedia Communications with a concentration in TV/Video Production. Kyle has volunteered for The Perfectly Flawed Foundation by working on website content/design, video shooting and editing, and helped with some of the fundraiser/awareness events that happen throughout the year. On his spare time he enjoys getting outdoors, traveling, fishing, photography/videography, and being with friends and family.


Matthew Klein

Matthew Klein graduated from Illinois Valley Community College with an Associate of Arts in May of 2020 and is currently a Junior studying Business Management and Leadership at Bradley University. Matthew has been an integral part in planning and executing multiple fundraising, community, and awareness events for The Perfectly Flawed Foundation. In his free time, Matthew enjoys flying drones, learning about technology, and spending time with his family and friends.
